The ONE Dental Plan is underwritten by Fidelity Security Life Insurance Company.  The description of benefits is as follows:

Preventive Services - (Class A) 100% Usual and Customary Rates, No waiting period.  Includes Exams, Cleanings, and Fluoride for children up to age 16. 

Basic Services - (Class B) 80% Usual and Customary Rates, 6-month waiting period.  Includes X-rays, Fillings, Simple Extractions.

Major Services - (Class C) 50% Usual and Customary Rates, 12-month waiting period.  Includes Endodontics, Oral Surgery (Surgical Extractions & Impactions), and Periodontics.

Major Services - (Class D) 50% Usual and Customary Rates, 24-month waiting period.  Includes Crowns, Bridges, and Dentures.

Deductibles:     Class A - $35 Lifetime, Class B, C, and D combined is $50 per calendar year

Miscellaneous: 12 month Rate Guarantee with optional 24 month guarantee, Calendar Year Maximum Benefit  $1,000

Effective Date: The plan effective date is always the first of the month following approval by Fidelity Security Life Insurance Company.
